SNMP question

I have a Synology DS415+ (not the host for the Domotz Agent in this case); I have looked at the default OIDs when adding an EYE and they don't seem to cover all the ones supported by the Synology box - I've downloaded their MIB files and, looking at them in a browser, I see ones such as for disk health (e.g. .1.3.6.1.4.1.6574.5.1.1.2) that I am interested in. I have added one as an EYE to test it, but what is not obvious is how to flag an alert if the status changes. Can you point me how to achieve that? Is there a document outlining the steps required?
